All patient forms and/or COVID consent must be completed online at
www.missionoralsurgery.com prior to appointment.

Please call our office when you arrive at the building. We will let you know if we are ready for your appointment. This allows us to minimize the number of patients in the office at one time.

Only one person is allowed in our office.
(with minors and assisted elderly patients as the exception)

When you arrive at our office, we will check you in, take your temperature and review your consent.

Masks are mandatory for entry. If you do not have one, please use the ones provided by us in reception.
